AB-67 Form (Change of Bus Service)
The AB-67 Form is used to submit transportation-related requests or updates for a student. This form allows parents or guardians to provide information or request assistance regarding student transportation. All submissions will be reviewed by the Transportation Department.
Please note:

This form cannot be used to request a change to a student’s existing bus stop location.

The form must be signed and submitted by the school administrator to be processed.

If the request is approved, a bus pass will be issued to the student.
