Welcome to Sawyer Road Elementary, a Marietta City School
ALL REGISTRATION IS COMPLETED ONLINE - That means parents can register their student(s) at their convenience on any device with internet access. Register your student and upload all needed documents from the comfort of your own home.
If you have already started the registration process but have not uploaded your documents, please log in and do so as soon as possible. You can use your smartphone or tablet to capture images of your documents using the feature called Mobile Capture. The system will walk you through the steps to upload these documents. Once you complete the process, you will be provided with a pdf file of your forms.
Please do not delay this final process. We can only approve your registration once all the documents have been received.

Required Student Documents
It is the parents' / guardians' responsibility to provide the required documentation upon enrollment. During the Gateway Registration process you were asked to upload the following required documents:
- Birth Certificate (student)
- Parent/Guardian Identification
- Hearing, Dental, Vision and Nutrition Screening Certificate (Georgia Form 3300)
- Certificate of Immunization (Georgia Form 3231) or Religious Objection
- Child’s Social Security Card or SSN Waiver
- Proof of Residency (Two Items)
- Report Card (if registering for Grades 1-5) - Report Card must indicate promoted or retained.
*If you live with a family member and the lease and/or rental agreement is not in your name, you must provide A Third Person Affidavit of Residency (signed and Notarized). You can find this form in Gateway or by clicking on the above link. You must turn in the Affidavit and all supporting documentation in order for us to accept it.
*Any Out-of-State health forms must be transferred to GA Health Forms. You can do this at the Cobb County Health Department (1738 County Services Parkway, Marietta, GA 30008 / 770-514-2300) or by a local pediatrician.
GA Form 3231 - Certificate of Immunization
Georgia Law requires children attending school (Kindergarten – 12th grade) to be age-appropriately immunized with all the required vaccines at the time of first entry into school. All students, regardless of grade and including foreign exchange students, must have the Georgia Department of Human Resources Form 3231 immunization certificate marked “Complete for School.”This document cannot have an expiration date prior to the first day of school or prior to your registration.
GA Form 3300
A student regardless of grade level, who has never been in a Georgia public school must provide certification of eye, ear, dental and nutrition examinations on the Georgia Department of Human Resources Form 3300. This document has four parts: (1) Vision, (2) Hearing, (3) Dental and, (4) Nutrition. All parts need to be filled out. It's okay to submit more than one form.Cobb County Health Department
