Choice Application Instructions
Current Choice Students:
Complete the MCS Enrollment Agreement (MCS Enrollment Agreement Spanish) and submit to your school by the 3rd Friday in December. If the form is not received by the 3rd Friday in December, you will forfeit your current Choice status and will attend your zoned school for the next school year.
New Choice Applicants:
- Complete the MCS Choice Application (MCS Choice Application Spanish) and the MCS Enrollment agreement (MCS Enrollment Agreement Spanish).
- Submit the completed PDFs together in one email to firstname.lastname@example.org by the 3rd Friday in December. Incomplete applications will not be considered for approval.
